Where the Wild Things Are is the twelfth episode of the fourth season and the 73rd overall episode of Grey's Anatomy.

Cristina is sleeping in the gallery. Izzie is sleeping on a gurney in the tunnels. Alex is sleeping on a bunk bed in an on-call room.

Meredith is sitting on her psychiatrist's couch, but she's not talking. Katharine says it's her third session, and she still hasn't talked. Meredith read a study that just going to psychiatrist is already benificial. Katharine wonders if she thinks that's the way to solve her problems. Meredith says she doesn't have problems. Katharine wonders what brings her here then.

George gets mad at the cockroaches in his new apartment before smashing them. Lexie is brushing her teeth and says the apartment is not that bad. George calls it the crapartment. Lexie says it's their home, they'll fix it up. George wonders with what money. Lexie says she likes it here, and she'd like it more if he liked it too. George smashes another cockroach.

Meredith tells Katharine that she's fine, but she hasn't been able to sleep. It's been five weeks. Katharine asks since what. Katharine says she works at the hospital, so she's heard gossip. Meredith says Derek is not what brought her here. Meredith gets paged and says she's got to go because she's in this contest.

Cristina and Izzie are both woken up by their pagers, but Alex sleeps through the sound of his.

Cristina, Izzie, and Meredith end up in the same hallway, all with the same destination. They continue to walk faster. Cristina bumps into Izzie, making her fall.

Richard tells Bailey, who's carrying around her baby, that the residents are engaged in a surgical contest with a points system, which is why they've been living in the hospital for two weeks. Bailey claims not to know what he's talking about as Izzie, Meredith, and Cristina come rushing past them. Bailey says they're just committed. Richard says it's a lion fight. Bailey says she's too busy to be a zookeeper and walks off to bring her baby to daycare. Nevertheless, Richard tells her to keep her animals under control.

Izzie, Meredith, and Cristina see Rose and Derek laughing and walking at the other end of the hall. They start trash-talking Rose. George comes up and wonders what Izzie did to her hair. Meredith says she knows they're trying to make her feel better, but the pausing just gives her a better shot to win. Meredith rushes off, followed by Izzie, George, and Cristina. Izzie tells George she cut her hair off to be sleeker. George can't believe she cut off her hair for a contest, but Izzie says living in the hospital is great. Cristina reminds her the contest is for residents only. Izzie has never been more on her game. Meredith's kind of sad that the contest is ending tonight, but Cristina is not. She wants the prize. Bailey won't tell them what the prize is, but they want it so bad.

Derek says goodbye to Rose and stays at the nurses station with Mark, who asks what he sees in her. He guesses it's sex, but then figures out they haven't slept together yet. Derek says Rose is waiting until it's serious. She has morals, and anticipation is better than sex.

Alex comes rushing in and asks the others why they didn't wake him. They put on trauma gowns and Cristina sees Callie and Erica coming in together. Cristina takes Callie aside and asks her about Erica. Callie says they went out and it got too late with all the dancing to go home, so they came straight to the hospital. Callie admits she's friends with Erica and walks off. Cristina tells the others that this is the downside of the contest. She should hang out with Hahn at her apartment as well. Izzie tells her to focus as it's much more fun to kick her ass when she's focused. They rush outside.

Alex has kept score and Yang is winning. Izzie is 26 points behind. Izzie's watched a lot of surgeries, but watching only gets you 3 points. Izzie says she can't make the attendings let her do procedures, but Cristina and Meredith say she's gotta ask and take charge. Cristina shows her how and opens the door of the arriving ambulance. The patient is a man found unconscious with a swollen ankle. Izzie takes the case and she suspects there's more to it. She sees a medical mystery, which will give her 80 points. Bailey comes outside and asks if only Izzie is working today. They all got paged for the ankle, so now they're just trolling for trauma cases. Bailey's got no one post-op or pre-op, so she tells them to get to work. A car drives into the ambulance bay and a woman shouts for help. The residents and Bailey rush to their aid. They were camping in the mountains and a bear attacked them. They drove all the way down as they didn't have time to wait for an ambulance. While Bailey checks on the driver's injured hand, Cristina and Alex help a man get out of the car to get him on a gurney, but as he stands up, intestines start to fall out of an open wound to the man's abdomen. Cristina manages to catch them before they fall the ground.

Scott says Phillip, the driver, put out his hand to touch a cub, but then the mother came. He doesn't know why Phillip did that, as they've gone out camping before. Erica and Richard come in and Bailey informs them on the complete evisceration. Richard orders supplies and Scott asks if they're going to put him under. Erica says it'll be the best thing that happened to him today.

Phillip asks if his brother is okay. All this is his fault, so he has to know if Scott is okay. Meredith and George tell him that they're taking him into surgery, but they don't know much yet. Callie says she can see right through his hand. Jennifer, who's standing against the wall, says the cub latched on and wouldn't let go. Then Scott came to help, the mother arrived. They can't believe they survived. Phillip keeps saying Scott's gotta be all right.

Izzie asks Otis if he remember why he fell. All he remembers is walking and the next moment, he was surrounded by paramedics. She asks if he's been feeling dizzy. He says maybe a little. She asks about other symptoms and he says yes, now that she mentions them. She asks if he's traveled. He goes back and forth to the East coast for his job. She asks if he's gotten any mosquito bites recently. He says all the time, because he lives near a lake. He thinks he has something bad, but she tells him there's no need to panic.

Alex, Cristina, Richard, and Bailey are scrubbing in. Richard says mama bear wasn't messing around. Erica comes in and says she's got another patient, but she wants to keep a close eye on his chest tube output and his blood pressure. Since Scott is at risk for overwhelming sepsis, they're going to take care of the life-threatening blood loss and get out of there. Hahn says she's in OR 4 and asks for hourly updates. Cristina will take care of that. Alex reminds her you don't get points for sucking up.

Lexie is putting supplies in a bag. George enters the supply room and says he hates the contest while Lexie keeps grabbing more supplies. Izzie enters the room and asks for supplies for her patient. She needs them fast because she's behind in the contest. George can't believe she'd get 26 points for a sprained ankle. Izzie says last time Callie treated what they thought was a sprained ankle, the patient had a perforated ulcer and died on the table. A medical mystery is 80 points and thus the holy grail of the competition. George says that sometimes an ankle is just an ankle though. Izzie doesn't believe this is just an ankle as there are some clues for something more. Lexie has found her supplies and Izzie leaves. Lexie notices the underpads and says they could make for placemats. George gives her a look and leaves, after which she sees a fruit bowl in a bedpan.

Phillip's done filling out his form, but it's illegible. Jennifer says he's a slob. Meredith's going to take Jennifer to the waiting room while George will come to take him to X-ray. Jennifer and Phillip are newlyweds. Phillip says she's his rebound girl. Jennifer says he's hopped up on painkillers right now, so he's oversharing. He rebounded her all the way to the church. They only knew each other for ten days, but when you know, you know. Jennifer kisses him and leaves.

Outside the room, she breaks down in crying. Phillip is a catch by anyone's standard so she's been waiting for the other shoe to drop, because a guy like that doesn't just marry a girl like her. She knows she must look crazy. Meredith says she's fine, but she noticed she's bleeding a little. Jennifer says she scratched herself on a branch when they were running. She takes off her cap, revealing a large scalp laceration.

Derek says that the CT is clear, but Mark says the laceration is deep. The bear clawed her good. Jennifer didn't even feel it. Mark says he has to repair the galea, but the reattachment shouldn't be a problem. Derek asks her to squeeze his hands. Jennifer says she's really happy because she just got married, so she hopes she doesn't have any permanent brain damage. Derek says it appears that she doesn't, but he asks Meredith page him if she has any neuro deficits. Mark says they can sew the scalp right back on and the hair should cover the scar. She won't even need surgery. Mark says Meredith will take care of her. Before leaving, Mark whispers to Meredith that Derek and Rose won't last.

Scott is hypothermic and his bowels are swollen, so they need to get him stabilized in the ICU and get him back to the OR when he's less acidotic. That will give them time to deal with the puncture wounds and the lacerations. Alex and Cristina clash over who gets to do the stitches, but Bailey tells Alex that he can do it because Cristina has to update Hahn hourly. Cristina says that doesn't take longer than a minute, so she's got 59 other minutes to do sutures. Bailey allows them to both do suture. Richard remarks there's a lot of enthusiasm for sutures. Cristina says she loves the fundamentals, but Bailey informs him it's a point per suture.

Lexie takes a framed picture from the wall in the hallway and walks off with it.

Callie says Otis has a hairline fracture. Izzie, who's putting on a cast, has run other tests which might indicate an inflammatory process going on. She doesn't want him to worry, but she asks if he's got someone to call as he maybe shouldn't be alone here today. Callie takes a look at his chart and asks Izzie to tap the fluid in Otis' knee to check for white cells or crystals and let her know if the tap is positive. She wishes Otis good luck before walking off. Otis gets scared when he hears he needs luck and says he wants to go. He wishes he had lived a healthier life and that he hadn't driven off his ex-wife, who was a saint, because now he's here, 60 and fat and alone and waiting for a young doctor to determine what horrible disease he's gonna die of.

Alex and Cristina rush past Callie on their way to the elevator. Cristina takes a moment to ask Callie since when she's been friends with her mentor. Callie says Erica is not her mentor and if Yang hadn't lived at the hospital for weeks, she would have noticed that she and Erica have been friends for a while. Cristina hopes Callie put in a good word for her, but Callie says they don't talk about her and then tells her Alex is getting on the elevator. Cristina rushes towards it and is just in time to get on it too.

Meredith and George are watching the scans. She wants to trade with George so that he can do the complex sutures on Jennifer's scalp, because Phillip reached out to a bear, which makes her think something bigger's going on than a broken hand. He married his rebound girl after only having known her for 10 days. Nobody does that. She thinks he has a brain tumor. George says he got married in the spur of the moment and he doesn't have a brain tumor. He'll take the scalp so that he doesn't have to see Meredith try to turn a broken hand into a neuro problem just to get some extra points, or to have a reason to talk to Shepherd. Meredith follows him outside and yells that she doesn't need a reason to talk to Shepherd. She'll just talk to him if she wants do. At that moment, she notices Katharine passing by.

As Lexie is stealing more supplies, Derek bumps into Rose, who's holding flowers she got from a patient. They flirt a little and he walks off. Rose puts the flowers down and tells Lexie she's trying to play it cool. As Meredith comes closer, Rose blurts out she loves him. She then realizes she doesn't know Lexie and walks off. Lexie takes the flowers and Meredith asks what's with the flowers. Lexie wants to know if she's asking as a resident or a sister. Meredith says the latter. Lexie says she's stealing stuff and asks if Meredith's okay. Meredith says she's a resident now and tells Lexie to stop stealing from the hospital.

Cristina has done 22 sutures, whereas Alex has only done 17. The patient is slowly improving. Richard has grown to like the competition and tells Alex Yang is winning. Alex says Cristina has to update Hahn. Cristina says Hahn can wait a few minutes, but Bailey makes her go. Bailey says she thought Richard told her to keep the animals under control, but Richard admits he likes to go to the zoo from time to time. 

Cristina enters Hahn's OR and tells her the patient is critical but stable. She gives some more facts and finishes. Hahn looks forward to another scintillating report in an hour. Cristina leaves and rushes back to Scott's room.

Mark suggests he and Derek have a sex contest themselves, because he thinks Derek needs some spice in his life. He's just trying to help. Derek says he's a worse human being for knowing Mark.

Izzie is draining fluid from Otis' back. He says he's alone and in pain and dying. Izzie tells him that by some lucky stroke of fate, he fractured his ankle and ended up at the hospital with her as a doctor. She's gonna find out what's wrong with him and fix it, and if she can't do it herself, she's gonna assemble a team of doctors that can. Then he can live a healthier life, maybe even call his ex-wife, and he's been given a second chance at life. This time, he can do it right. She then continues her test. He screams in pain.

Meredith takes Phillip into Scott's room. Scott recognizes him and says he feels bad. He wondered what made Phillip touch a cub. Phillip says he wasn't thinking. He's so sorry. Meredith likes to hear that Phillip knew better than to touch the cub. Scott says he's having a midlife crisis, he even married it. Phillip says the situation doesn't give him the right to talk smack about his wife. Meredith asks if the spontaneous wedding was out of character for him. Cristina says Scott needs rest. Meredith takes Phillip away. Cristina is at her 59th suture, but it's time for her update again. Cristina runs out, but Richard says there's no running allowed in the hospital.

George is suturing up Jennifer's scalp in the ER as Bailey checks on Izzie. She wonders why they're doing a spinal tap on an ankle fracture. Izzie says she's doing a full work-up. Bailey asks how many points and asks her to tell her that she's not putting the man through painful and unnecessary tests just to win the contest. Izzie lists some tests, symptoms and possible diagnoses that justify her actions. She does want to win the contest, but she also wants to make sure that she's not missing anything that will only be discovered when it's too late. Bailey tells her to carry on. Izzie takes off and George walks up to her. He says when you hear hoofbeats, think horses, not zebras. She says exactly.

Meredith is checking up on Phillip. She asks if the illegible handwriting and squinting at paperwork is normal for him. Phillip says he never was a straight A student. Jennifer is sitting with him and says they both need rest. Meredith wants one final test. She place her fingers behind his head and moves them towards herself, asking him to tell her when he can see her fingers. He says it way too late to be normal and she cheerfully screams it's a tumor. She apologizes.

Phillip thinks Meredith is just making up the tumor so he has an excuse as to why he reached out to touch the cub. She says there are other symptoms as well, impulsivity included. Jennifer says like he married a trashy waitress. Meredith says she just wants him to have an MRI so they can be sure, but he says no. Jennifer tells him to have it, because she is convinced he really loves her.

Cristina gives Hahn a quick but thorough update. Scott is doing better. Cristina wants to leave, but Erica wants to know if she's racing because of the contest. Cristina says yes and Hahn asks if she's winning. Cristina says yes, if she can get back to doing her sutures. Hahn says that's too bad, because she was gonna let Cristina watch her put in the new valve, but it's okay if the contest is more important. Cristina puts on her mask and says she'd be happy to watch.

George finds Izzie swearing at Otis' test results. All tests are clear and she wonders what she's missing. George takes a look and says she's missing the fact that both she and Meredith are doing unnecessary medical tests to win a stupid contest. Izzie says great doctors just know when there's something more. George says great doctors know when to stop.  Izzie says her instincts are bad lately and she's in danger of going extinct because of her bad instincts, so she has to win this contest.

Meredith has paged George to show him Phillip's scans, revealing a big brain tumor. She's not requiring an apology from him because saving Phillip's life is rewarding enough for her. Meredith admits she needs a favor.

Alex is finishing up his 87th suture. Richard says he's an underdog. He has the skills, but he's scrappy and a fighter. Richard always roots for the underdog. Alex says he's not an underdog. He compares himself to Scott, who's fighting for his life, suffering from the mistakes that other people made. It makes him have to work harder to overcome the cards he got dealt. Scott then crashes and they call a code blue.

George is showing Derek the scans. Meredith made him show them to Derek and Derek figures out that Meredith didn't want to face him herself.

Alex is performing CPR.

Richard tells Jennifer and Phillip that Scott has died. Phillip starts laughing and Jennifer asks Meredith about the scan. She says the neurosurgeon is gonna come in soon, but Jennifer needs to know now. Meredith admits she was right about the tumor. Phillip has a malignant glioma, which causes strange behavior. They can't operate as it's too invasive. The tumor has been growing for a long time. Meredith says she's sorry. Phillip finally stops laughing and says he deserved that for killing his brother. He's crying now.

Jennifer follows Meredith outside the room. She thinks she's just a symptom of the tumor. Meredith can't know that, as only Jennifer herself can know that for sure.

Izzie tells Otis that all the test results are negative. He asks about the tests that showed elevated levels of something. Izzie admits that these tests were nonspecific so they just indicated that he probably has a relatively minor flu. He can't believe he spent the entire day getting put through tests for the flu. Izzie says it seemed like there something more. Izzie knows it's hard to see the silver lining here, but she tells him to think about the discoveries he made today about himself, his life, and his ex-wife. Otis screams his ex-wife was a castrating bitch and he only missed her today because if they were still together, she would have shot him a long time ago and then he wouldn't have been in the hospital all day. Izzie says she didn't mean to put him through all this. He demands his prescription so he can finally get out of here. Izzie says the flu doesn't respond to antibiotics so she suggests plenty of fluids and rest. He starts screaming angrily. She repeats she's sorry multiple times and walks off.

The residents are watching Bailey with her son in the conference room. Alex says Cristina only got 3 points for watching the surgery, but Cristina trusts they all screwed up enough that it won't matter. Izzie thinks she could still win. Bailey has finished counting the points and comes out of the room. The winner is Meredith. She received 80 points for finding the tumor. They all congratulate her non-enthusiastically. The prize is a sparkle pager, which has been passed down from resident to resident. For the next three months, whenever any one of them gets a surgery, they have to page the sparkle pager and Meredith has the right to take the surgery away from them. Bailey tells Meredith to enjoy the power and then tells them all to go home, as they need to wash up and she's tired of looking at their faces. Meredith says she thought she'd enjoy the prize more, but she doesn't want to give it to someone else. She walks off.

Mark tells Derek he doesn't like Rose. If Derek were sleeping with her, he could understand why Derek was spending time with her, but they're not sleeping together. Mark thought it was just going to be the two of them after Addison and Meredith, and he's been patiently waiting for that. He wanted to go on the hunt for women together with Derek. Derek thinks Mark has trouble getting laid, but Mark confesses Derek is the only person he can talk too. Mark asks Derek if he's serious about Rose. Derek says he could be. Mark then decides to give Rose a chance, but he says he's a lesser man for knowing Derek. Derek says Mark can't steal his line.

Richard finds Izzie catching up on paperwork in the clinic. He knows Izzie was supposed to be off duty an hour ago, but he figures she needs to catch up on the paperwork that piled up during the contest. He also heard about Otis' case and the $120,000 worth of unnecessary medical tests she ordered. Izzie admits she lost and she put a man through hell for no reason. She was a bad doctor with bad instincts today, so she deserved to lose. She doesn't even know why she bothered competing. Richard concludes he doesn't need to yell. Richard tells her that in his year, he lost by two points. The contest is a lion fight, so she has to celebrate her wounds. The scars she bears are the sign of a competitor. She was in a lion fight, and just because she lost doesn't mean she doesn't know how to roar. Izzie smiles and they wish each other a good night.

Cristina comes home to Callie and Erica drinking wine in the living room. Cristina tells Erica it's an honor to have her here. Cristina notices she's disturbing their conversation and heads into her bedroom, but she can't help but peek from the bedroom.

George comes home and finds the apartment decorated with supplies from the hospital. Lexie is waiting for him and tells him to hear her out. She knows the apartment is not much and everything is stolen from the hospital. He can report her from stealing because she's a thief, but she thinks more of it as nesting. She's nesting for him, because he hates it here. She knows he wants to be at Meredith's house, where it's warm and where he has friends. She says this apartment can be warm too and she put out traps for the roaches. Also, she can be his friend because she doesn't really have friends except him and she doesn't have a home to go to anymore except here. George realizes he complains a lot and decides to stop doing that. He's gonna focus on what's good, like this apartment. They sit down on the couch. He wishes she'd stolen a TV. She says she tried, but they were bolted to the wall.

Meredith is doing research at the hospital. The sparkle pager goes off, but she ignores it.

The next morning, Meredith sees Derek and Rose kissing before they part ways. Derek walks up to her and greets her. Meredith tells him she may have found a way to save people with the kind of tumor that Phillip has. She did all this research and she found this rare method that's never been tested on humans, but she thinks she can find patients for a clinical trial. She could do the lab work and track the results, but she's gonna need a neurosurgeon, and he's one of the best. It's not about them and she would go somewhere else if she could, but she thinks she has found a way to save lives. Derek looks at her and her research and as Rose watches them from another nurses station, he nods.

Meredith is in Katharine's office again. Meredith decides to start talking now. She watches the fish in the fish tank.

Izzie believed that it was a medical mystery as he was found unconscious, so she ran several tests, but they revealed nothing except for a relatively minor flu. She suggested that he get a lot of fluids and rest.

Scott came into the ER after being mauled by a bear. His intestines were falling out of his abdomen. He was taken into surgery. After surgery, Alex and Cristina stitched up his wounds. Scott later coded and he was unable to be resuscitated.

Phillip came into the ER after being mauled by a bear. He had a serious injury to his hand. He was taken for x-rays. Callie said his hand needed surgery, but he needed 24 hours of antibiotics first, so Meredith irrigated the wound and splinted it. When Meredith found out that he had provoked the bear and had other signs of impulsive behavior, she checked for a brain tumor. An MRI confirmed her suspicions and Derek informed Phillip that surgery was not an option for the type of tumor he had. However, Meredith did additional research and found a solution she thought might work for him and for other patients.
