Archer has neurocysticercosis - parasites in his brain. Derek initially believes the surgery to be impossible, even consulting with surgeons at Mayo Clinic and Cedars-Sinai Medical Center, though after a pep-talk from Sam, he believes he is able to do so.

  • Removal of the "perfect tumor" (Give Peace a Chance): Derek Shepherd removed a large and highly vascularized tumor from the spine of Isaac, a lab tech at Seattle Grace Mercy West. The operation took thirty hours and took a physical toll on both Derek and his resident, Jackson Avery.
